New Patient Registration
Please visit the surgery to register and bring your NHS medical card with you. If you do not have a medical card we will provide the relevant forms for you to complete.
Please allow 48 hours before making an appointment as this gives us time to complete necessary administration.
Please note you will need to provide two forms of identity; one to confirm your name and identity, and one to confirm your address.
When you register you will also be asked to fill out a medical questionnaire. This is because it can take a considerable time for us to receive your medical records.
Alternatively you may print off a registration form, fill it out and bring it in to reception.
Change of Name / Address
Please inform us as soon as possible of any changes such as name and address. If you move outside of our practice boundary it will be necessary to register with a different doctor closer to your new home.
